Before signing your application, you will be asked to review and accept the terms and conditions stated in the Foundation's Guidelines for Using Your Charitable Checking Account brochure.

Both forms and the brochure are in Adobe Acrobat PDF format. If you do not have Acrobat click here to download a free version. Once installed, you can save the form or brochure to your hard drive. Simply right-click your selection, select "Save Target As" from the File menu, then locate a destination directory on your hard drive.
